Win a $5000 Holiday Draw Signup

• Enter customer details here to enter them in the draw for the $5000 travel voucher prize.
• They will get another chance to win for every $100 they spend (so we need the order value minus pills).
• Qualifying purchases include eveything but pills.
• We must get an email address to contact the winner.
• We must get the Tr# so we can validate the winner.
• We must get your Store# so we know where the entry came from.
• Phone number and/or name is useful but not nessisary.
• If we can't get in touch with the winner after two weeks another winner will be drawn.
• Customers will be subscribed to our marketing email database as a condition of entry.
• Peaches & Cream staff and their families cannot enter.
• Please ask them where they heard about the competition and select the checkbox at the bottom that best matches. This is useful information for getting more people in store.

NOTE: If they spend $500 they will get 5 chances to win in the draw. This is why we need the order value. DO NOT enter the same customer multiple times.
